Learning needs log and teacher meeting sheet
Write down what you see at home for two or three weeks, and what the school tells you. Bring this page to the next meeting with the form teacher or SEN Officer.
Download the PDF (A4, 2 pages)Open to print
Use it with: Subject levels at P4 · Learning support in primary school
This page stays with you. It is not a test and it does not diagnose. Write what happened, not a label. Keep it at home or in your child’s folder.
1What we see at home
One line each time. For example: reading aloud, spelling, sums, homework time, mornings, friends or feelings.
For each time something was hard, note the date, the task, what happened, how long it lasted and what helped.
2What the school told us
Note the date, who told you (form teacher, subject teacher, SEN Officer), what they said and any support offered, such as LSP, LSM or the dyslexia programme.

Fill in part 3 before the meeting, ask the questions in part 4, and write down what you agree in part 5.
3Before we meet
4Questions to ask
- What do you see in class? Is it the same as at home?
- Is my child in any support now: LSP (English), LSM (Maths), the Mother Tongue Support Programme or the dyslexia programme?
- Can we meet the SEN Officer? Can a MOE Educational Psychologist advise the school?
- Should we get a professional assessment? Which reports would help the school?
- P4: which subject levels do you recommend, and when is the option form due?
- P5 or P6: should we apply for PSLE access arrangements, and which reports do you need?
- What can we practise at home, and for how long each day?
5What we agreed
